HIPAA Notice of Privacy Practices

Maddox Nutrition LLC is committed to protecting the privacy of your health information. This Notice explains how we may use and disclose your protected health information (PHI), your rights regarding your PHI, and our responsibilities under applicable privacy law.

Your Health Information and Your Rights

You have the right to:

Obtain a copy of your health information

You may request access to your health information and may request a copy of records maintained by Maddox Nutrition, subject to applicable law.

Request correction

You may ask us to correct health information that you believe is inaccurate or incomplete. We may deny a request when permitted by law, but we will provide you with a written explanation when required.

Request confidential communications

You may ask us to contact you in a particular way or at a particular location. We will consider reasonable requests as required by law.

You may ask us to limit how we use or disclose your health information. We are not required to agree to every request, except where applicable law requires a restriction. You may request a restriction on disclosure of PHI to a health plan when the restriction requirements under HIPAA are met.'

You may complain to Maddox Nutrition or to the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.

For example: we may communicate with another healthcare professional involved in your treatment when permitted by law.

For example: we may provide information to your health insurance plan when submitting or appealing a claim.

We may use and disclose health information for activities necessary to operate our practice, improve care, and manage our services.

As Required by Law

We may use or disclose health information when required by federal, state, or other applicable law.

Public Health and Other Legally Permitted Purposes

We may disclose health information for certain legally permitted purposes, including public health activities, health oversight, law enforcement, judicial or administrative proceedings, workers’ compensation, and other purposes permitted by HIPAA. We will disclose only the information permitted or required for the applicable purpose.

Uses and Disclosures Requiring Your Authorization

Certain uses and disclosures require your written authorization unless another legal exception applies. These may include:

  • certain disclosures of psychotherapy notes, when applicable;

  • marketing communications requiring authorization;

  • sales of PHI requiring authorization; and

  • other uses or disclosures not otherwise permitted by applicable law.

You may revoke an authorization in writing at any time, except to the extent that Maddox Nutrition has already acted in reliance on it or another legal exception applies.

Communication With People Involved in Your Care

When permitted by law, we may disclose limited health information to a family member, close personal friend, or other person involved in your care or payment for your care when appropriate. Electronic Communication

We may communicate with you electronically when appropriate. We encourage you to use secure communication methods provided by the practice for clinical information. Email and text messaging may carry privacy or security risks.

Our Responsibilities

Maddox Nutrition LLC is required by law to:

  • maintain the privacy and security of your PHI;

  • provide you with this Notice describing our legal duties and privacy practices;

  • notify you as required by law following a breach of unsecured PHI;

  • follow the privacy practices described in this Notice while it is in effect; and

  • provide you with a copy of this Notice upon request.

We reserve the right to change our privacy practices and this Notice as permitted by law. Any revised Notice will be made available as required by law.

Specially Protected Information

Certain types of health information may receive additional protection under federal or state law. This may include information related to substance use disorder treatment and other categories of specially protected information. When applicable, Maddox Nutrition will comply with additional legal requirements governing such information.
