7. Scheduling, Cancellations, and No-Shows
Appointment scheduling, client intake, and related communications are managed through our secure EMR system or other designated HIPAA-secure tools. By scheduling an appointment, you agree to the following:
Cancellations
We ask that you provide at least 24 hours' notice if you need to cancel or reschedule an appointment. This allows us to offer the time to another client since your session time was reserved exclusively for you.
Late Cancellations and No-Shows
Late cancellations and no-shows may be subject to fees as disclosed in our client policies and scheduling materials. This fee is not covered by insurance and will be charged to the payment method on file.
Telehealth Appointments
For virtual appointments, please ensure you are in a private, stable-connection environment at the time of your session. If you experience technical difficulties, please contact us as soon as possible.
8. Payment Terms
Fees for services are disclosed prior to scheduling. Payment is processed through Stripe, our secure payment processor, integrated within our EMR system. By providing payment information, you authorize us to charge the agreed-upon fees for services rendered.
Maddox Nutrition LLC may participate with various insurance plans and networks. Participation, eligibility, and coverage are subject to change. Current insurance information will be provided during scheduling or intake.
We also welcome private pay clients.
For clients using out-of-network benefits, superbills may be provided upon request for possible reimbursement through your insurance provider. Please note that reimbursement is not guaranteed and depends on your individual insurance plan and benefits.
Clients are responsible for understanding fees, insurance coverage, copays, deductibles, coinsurance amounts, and payment obligations prior to services.
